The QuitCash Challenge™ Sweepstakes Rules

Sweepstakes Sponsor: ClearWay MinnesotaSM and QUITPLAN® Services ("Sponsors"), Two Appletree Square, 8011 34th Avenue South, Suite 400, Minneapolis, MN 55425

The Entry Period for The QuitCash Challenge (the "Sweepstakes") starts at 7:00 a.m. CDT August 10, 2009, and continues until 11:59 p.m. CDT September 30, 2009 ("Entry Period"). The Sweepstakes requires participants to remain tobacco-free from October 1, 2009, at least through the Sweepstakes Closing Date of October 28, 2009 ("Sweepstakes Period"), and as long thereafter as needed for tobacco-free status to be verified.

  1. Entry: No purchase necessary. Limit one entry per person/email address/mobile phone number regardless of method of entry. To enter:
    1. Complete registration form:
      • Online at www.quitcash.com between 7:00 a.m. CDT August 10, 2009, and 11:59 p.m. CDT September 30, 2009.
      • Mobile registration by texting "QuitCash" to 38681 to receive a link to a registration form. Standard texting fees apply.
      • By mail: Send a self-addressed stamped envelope to The QuitCash Challenge™ Sweepstakes, postmarked no later than September 30, 2009, to: ClearWay MinnesotaSM, 8011 34th Avenue South, Suite 400, Minneapolis, MN 55425
    2. Sweepstakes entrants must quit tobacco on or after September 1, 2009, and remain tobacco-free from October 1, 2009, through and including October 28, 2009. Tobacco-free means no use of tobacco products of any kind, including electronic or e-cigarettes. Entrants are permitted to use nicotine replacement therapies (e.g. patch, gum, inhaler or lozenge and/or prescription medication). Tobacco-free status will require verification by urine test.
  2. Eligibility: Sweepstakes is open to residents of Minnesota who are 18 years or older at the time of entry. Entrants must also be current daily tobacco users (cigarettes, cigars, pipes, chewing tobacco). Sweepstakes entrants will be required to sign a statement verifying their smoking status, signed by a witness. Current employees of Sponsor, their immediate families and household members, and their respective advertising and promotional agencies are not eligible. Entries will be rejected if registration form is not fully completed and submitted by the dates and times listed above. Illegible and incomplete entries will be void. Contest is void outside Minnesota and where prohibited, and subject to all applicable laws.
  3. Tobacco-Free Friend: Entrants may sign up with a Tobacco-Free Friend for support, but it is not required. Tobacco-Free Friend eligibility: Friends must be residents of Minnesota who are 18 years or older and tobacco-free. Registered Friends are eligible for a Tobacco-Free Friend Prize, subject to terms and conditions herein.
  4. Drawing and Awarding of Prizes: Random draws to select the potential winners from all eligible entries received will be conducted by a representative of the Sweepstakes Sponsor on October 28, 2009, at approximately 10:00 a.m. CDT in Minneapolis, Minnesota. The first selected individual entrant will be eligible to win the Grand Prize. Following the Grand Prize draw, first, second and third place prizes will be drawn from remaining eligible entries. Tobacco-Free Friends who are registered with prize-winning entrants will be eligible to win a Tobacco-Free Friend Prize. Odds of winning any Prize depend on the total number of eligible entries received. Selected entrants will be contacted immediately by email at the email address indicated on their entry form or by telephone at the telephone number indicated on their entry form. If a selected entrant cannot be reached within three days of being selected or if the selected entrant declines the prize, or does not otherwise comply with these Sweepstakes Rules, all related prizes will be forfeited and Sponsor may conduct another random draw in the same manner to select a replacement entrant. Each potential winner will be required to sign and return to Sponsor, within five (5) days of the date notice or attempted notice, an Affidavit of Eligibility and Liability/Publicity Release in order to claim his/her prize.
  5. Winner Verification/Urine Test:Potential winners (including their Tobacco-Free Friends) may be requested to provide proof of identification and eligibility. Tobacco-Free Friends or another witness will be asked to confirm in writing that eligible Sweepstakes entrants who are potential winners have in fact remained tobacco-free for the Sweepstakes Period and as long thereafter as needed for tobacco-free status to be verified and were in fact tobacco users for the year prior to entering the Sweepstakes. Each selected entrant will be required to take a urine test within five (5) days of being notified. The urine tests will be conducted by the University of Minnesota, to verify their tobacco-free status. By entering the Sweepstakes, all entrants agree to be bound by the results of the urine test. Entrant specifically acknowledges and agrees that Sponsor must rely on the urine test to determine whether or not a potential winner is tobacco-free, regardless of the possibility that testing methods and results are not always 100 percent accurate or conclusive. All decisions of Sponsor and testing firm are FINAL. Potential winners will need to remain tobacco-free until after the testing results are evaluated and tobacco-free status is confirmed (which may take up to a couple of weeks after the Sweepstakes Closing Date). Please note: we recommend all entrants remain tobacco-free at least until after a winner is announced in the event that a re-draw is required for prizing.
  6. Identification: If the identity of an online entrant is disputed, the authorized account holder of the email address submitted at the time of entry will be deemed to be the entrant. The individual assigned to the email address for the domain associated with the submitted email address is considered the authorized account holder. If the identity of a text entrant is disputed, the authorized account holder of the related services account at the time of entry will be deemed to be the entrant. A selected entrant may be required to provide proof that he/she is the authorized account holder associated with the selected entry. All electronic entries must be submitted from a valid and verifiable account (email or text), validity to be determined solely at Sponsor discretion. The sole determinant of time for the purposes of receipt of a valid online entry in this Sweepstakes will be the Sweepstakes Sponsor’s server machine(s).
  7. Liability/Publicity: Potential winners of any prize must sign an Affidavit of Eligibility and Liability/Publicity Release and IRS form W-9 within five (5) days of being notified before being named a winner, failing which they will forfeit their prize. Winners agree that their name, likeness, city of residence and/or biographical information may be used without limitation by Sponsor for publicity, promotional and/or advertising purposes (except where prohibited by law) in any medium throughout the world without additional compensation or right of approval.
